&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;This topic first came up during our opening keynote panel on Thursday with Professor &lt;A href="http://economix.blogs.nytimes.com/2009/06/12/the-cost-of-health-care-reform/" mce_href="http://economix.blogs.nytimes.com/2009/06/12/the-cost-of-health-care-reform/"&gt;&lt;FONT color=#0000ff&gt;Uwe E. Reinhard&lt;/FONT&gt;&lt;/A&gt;, &lt;A href="http://www.thehealthcareblog.com/the_health_care_blog/2008/09/will-we-need-a.html" mce_href="http://www.thehealthcareblog.com/the_health_care_blog/2008/09/will-we-need-a.html"&gt;&lt;FONT color=#0000ff&gt;Dr. David Kibbe&lt;/FONT&gt;&lt;/A&gt; and former HHS Secretary &lt;A href="http://en.wikipedia.org/wiki/Mike_Leavitt" mce_href="http://en.wikipedia.org/wiki/Mike_Leavitt"&gt;&lt;FONT color=#0000ff&gt;Mike Leavitt&lt;/FONT&gt;&lt;/A&gt;.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Secretary Leavitt introduced the notion that advancements in reimbursement reform are needed as much as stimulus for technology adoption.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;According to Leavitt, health IT will become ubiquitous when consumers demand it from their healthcare providers.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;While I agree with Levitt about informed, engaged consumers being key drivers of change, I also think that physicians, industry and other partners must demand a smarter approach to reimbursement from policymakers.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;One place to begin innovating and experimenting with a better reimbursement system is at the state level.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;During this morning’s keynote, &lt;A href="http://www.chcf.org/aboutchcf/view.cfm?itemID=108866" mce_href="http://www.chcf.org/aboutchcf/view.cfm?itemID=108866"&gt;&lt;FONT color=#0000ff&gt;Dr. Mark Smith&lt;/FONT&gt;&lt;/A&gt; of the California Health Care Foundation, made several insightful observations about factors affecting HIT adoption such as;&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;better, simpler solutions for physicians, a stronger voice for government as purchaser in mandating effective standards and a reimbursement policy that rewards outcomes vs. volume.&amp;nbsp; However, as &lt;A href="http://healthblawg.typepad.com/healthblawg/2009/06/peter-neupert-and-the-latest-on-microsoft-healthvault.html" mce_href="http://healthblawg.typepad.com/healthblawg/2009/06/peter-neupert-and-the-latest-on-microsoft-healthvault.html"&gt;&lt;FONT color=#0000ff&gt;David Harlow&lt;/FONT&gt;&lt;/A&gt; pointed out during a conversation today, our tough economic climate makes it difficult for states to take the lead.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;Dr. Kibbe emphasized the need for web-based tools that could be assembled in a modular way to qualify for stimulus dollars.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Both Kibbe and Smith made the strong point that physicians haven’t adopted existing technology solutions for a reason and that any stimulus/policy reform needs to be flexible enough to allow innovation in solutions vs. trying to “jam” solutions that don’t meet current needs or practical market requirements. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;In speaking with our Amalga partners, they too are interested in understanding how meaningful use and certification will be defined and applied.&amp;nbsp; Our customers shared many examples with each other of liberating and unifying data which makes measuring quality and proving effectiveness easier and automatic.&amp;nbsp; As much as health IT has the potential to help cut inefficiency and improve operational throughput, we still need the &lt;A href="http://blogs.technet.com/neupertonhealth/archive/2009/06/09/and-the-debate-rages-on-about-access.aspx" mce_href="http://blogs.technet.com/neupertonhealth/archive/2009/06/09/and-the-debate-rages-on-about-access.aspx"&gt;right policy solutions&lt;/A&gt; to bring greater transparency to what patients are purchasing from their providers and that quality of those services…as Leavitt demonstrated humorously with his story about &lt;A href="http://blogs.wsj.com/washwire/2007/02/28/leavitt%E2%80%99s-colonoscopy-a-teaching-moment/" mce_href="http://blogs.wsj.com/washwire/2007/02/28/leavitt%E2%80%99s-colonoscopy-a-teaching-moment/"&gt;colonoscopy pricing&lt;/A&gt;.&lt;/SPAN&gt;&lt;/P&gt;

&lt;P class=MsoNormal style="MARGIN: 0in 0in 0pt; LINE-HEIGHT: normal"&gt;&lt;SPAN style="FONT-SIZE: 10pt; FONT-FAMILY: 'Arial','sans-serif'"&gt;Real-time, accessible, meaningful and comprehensive data is fundamental to health care as a whole–to make a diagnosis, provide quality care, pay the right bill, discover new therapies, and so on.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;What’s of paramount importance is liberating the data and making it available for re-use in different contexts.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;This is critical for improving outcomes, paying for value, creating a learning healthcare system, enabling discoveries and fundamentally changing the dynamics of the ecosystem.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;We should be treating health data as a vital asset—health enterprises and consumers—to drive an efficient, high-quality, value-based, evidence-focused future for medicine.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;

&lt;P class=MsoNormal style="MARGIN: 0in 0in 0pt; LINE-HEIGHT: normal"&gt;&lt;SPAN style="FONT-SIZE: 10pt; FONT-FAMILY: 'Arial','sans-serif'"&gt;&lt;o:p&gt;&amp;nbsp;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 0pt; LINE-HEIGHT: normal"&gt;&lt;SPAN style="FONT-SIZE: 10pt; FONT-FAMILY: 'Arial','sans-serif'"&gt;First, there is the “it’s-my-data” wall put up by hospitals, insurance plans, pharmacy benefit managers, and others. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;They believe there is some competitive advantage by keeping the data inside their walls.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;And there are lots of excuses supporting their position–patients don’t want it, they can’t understand it, it might do them harm blah, blah, blah. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;This wall is starting to crack.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Many institutions recognize that the consumer has a right to a copy of their data and are making the appropriate connections to personally controlled health data repositories like &lt;A href="http://www.healthvault.com/Personal/index.html" mce_href="http://www.healthvault.com/Personal/index.html"&gt;HealthVault&lt;/A&gt; or &lt;A href="https://www.google.com/accounts/ServiceLogin?service=health&amp;amp;nui=1&amp;amp;continue=https%3A%2F%2Fwww.google.com%2Fhealth%2Fp%2F&amp;amp;followup=https%3A%2F%2Fwww.google.com%2Fhealth%2Fp%2F&amp;amp;rm=hide" mce_href="https://www.google.com/accounts/ServiceLogin?service=health&amp;amp;nui=1&amp;amp;continue=https%3A%2F%2Fwww.google.com%2Fhealth%2Fp%2F&amp;amp;followup=https%3A%2F%2Fwww.google.com%2Fhealth%2Fp%2F&amp;amp;rm=hide"&gt;Google Health&lt;/A&gt;.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Many others are writing about it too—John Moore asserted in a recent post, “&lt;A href="http://chilmarkresearch.com/2009/03/26/siloed-tethered-phrs-are-a-dead-end/" mce_href="http://chilmarkresearch.com/2009/03/26/siloed-tethered-phrs-are-a-dead-end/"&gt;&lt;I style="mso-bidi-font-style: normal"&gt;Personal health data belongs to the consumer and the consumer should decide how it is shared. This is a very radical concept that still has most providers, payers and other data holders shaking in their boots.” &lt;/I&gt;&lt;/A&gt;&lt;I style="mso-bidi-font-style: normal"&gt;&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;/I&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 0pt; LINE-HEIGHT: normal"&gt;&lt;SPAN style="FONT-SIZE: 10pt; FONT-FAMILY: 'Arial','sans-serif'"&gt;&lt;o:p&gt;&amp;nbsp;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 0pt; LINE-HEIGHT: normal"&gt;&lt;SPAN style="FONT-SIZE: 10pt; FONT-FAMILY: 'Arial','sans-serif'"&gt;The second wall is the “waiting-for-the-right-standards-set-by-government” wall.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;There are multiple excuses buttressing this wall; the core of which come down to technology, standards or policy excuses.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Without debating each point–one inexcusable barrier is the IT enterprise system vendors who make it difficult or expensive to get access to the data, to separate it from the application. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;They believe that proprietary “lock-in” provides them with a strategic advantage.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;We’re all in business and need to create strategic value for our products, but let’s do it in the application layer—rules, workflow, user experience, price, or services—not by trapping patient data in a proprietary database structure.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;

They have to be able to build cross-vendor systems to improve care.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;If information can be made liquid—flowing from where it is generated to where it is needed, and combining it with other bits of information to provide a comprehensive view—it can be tremendously powerful.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;

&lt;P class=MsoNormal style="MARGIN: 0in 0in 0pt; LINE-HEIGHT: normal"&gt;&lt;SPAN style="FONT-SIZE: 10pt; FONT-FAMILY: 'Arial','sans-serif'"&gt;We, in the IT industry, can step up and be a driving force in enabling data to become liquid—specifically, doing this by &lt;A href="http://blogs.technet.com/neupertonhealth/archive/2009/01/12/before-you-finalize-your-health-it-shopping-list.aspx" mce_href="http://blogs.technet.com/neupertonhealth/archive/2009/01/12/before-you-finalize-your-health-it-shopping-list.aspx"&gt;separating data from applications&lt;/A&gt;. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;This is one of the recommendations from a study by the National Research Council of the National Academies that takes a look at what types of &lt;A href="http://books.nap.edu/openbook.php?record_id=12572&amp;amp;page=R1" mce_href="http://books.nap.edu/openbook.php?record_id=12572&amp;amp;page=R1"&gt;computational technology&lt;/A&gt; and investments are best for improving health outcomes. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Let the excuse not be that the data is trapped in systems that we built, that we have to wait for standards.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;

&lt;P&gt;There is much to report from the event -- because there were a lot of smart and thoughtful panelists -- and because the issues are at the same time complicated, familiar and not super well understood.&amp;nbsp;&amp;nbsp;&amp;nbsp; I don't have time to cover them all in this post.&lt;/P&gt;
&lt;P&gt;There is a strong consensus (it showed up in comments from many panelists) around the theme I care most about -- which is we must focus on the outcomes one wants to achieve (better health outcomes at the same or lower cost) and then encourage innovation between providers and technology suppliers to deliver those outcomes.&amp;nbsp;&amp;nbsp; Said another way -- technology is a tool (a means) and not an end in itself.&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; This is why health reform and the health stimulus have to be intertwined and self-reinforcing.&amp;nbsp;&amp;nbsp; Just spending money on HIT is not going to lead to the 'down payment' (i.e. future savings) that Obama believes it is.&lt;/P&gt;
&lt;P&gt;&lt;A class="" href="http://www.chcf.org/topics/healthinsurance/index.cfm?itemID=123739" mce_href="http://www.chcf.org/topics/healthinsurance/index.cfm?itemID=123739"&gt;Mark Smith&lt;/A&gt; brought this to life most effectively in his remarks -- that by adding a computer to Kramer's bookstore you don't end up turning it into Amazon.com.&amp;nbsp;&amp;nbsp; &lt;A class="" href="https://www.kaiserpermanente.org/" mce_href="https://www.kaiserpermanente.org/"&gt;Kaiser&lt;/A&gt; reported in the article summarizing the experience in Hawaii with KP Healthconnect (EMR) -- that family practice office visits went down by 25+%.&amp;nbsp;&amp;nbsp; For Kaiser, an integrated delivery network and health plan -- that is a benefit to their bottom line. (I should note patient satisfaction went up in this period because needs were met by phone/email -- a totally win/win situation for Kaiser and consumer alike).&amp;nbsp; For a more typical, small office family physician -- that is a disaster...because revenues are tied to visits.&amp;nbsp;&amp;nbsp;&amp;nbsp; To succeed, we have to change the paradigm of healthcare away from the tyranny of the visit based payment system (Kramer's bookstore) and reward longitudinal care systems (future amazon.com).&amp;nbsp;&amp;nbsp;&amp;nbsp; If we spend a bunch of money automating existing systems of care -- and not enabling new platforms -- we have missed the huge opportunity to make a difference.&lt;/P&gt;
&lt;P&gt;&lt;A class="" href="http://www.markle.org/about_markle/management/carol_diamond.php" mce_href="http://www.markle.org/about_markle/management/carol_diamond.php"&gt;Carol Diamond&lt;/A&gt; made the subtle, but I think hugely important point that has not fully sunk in to the folks waiting at the starting line of the HIT land rush -- that the public is going to expect accountability for improved outcomes with the stimulus spend.&amp;nbsp;&amp;nbsp;&amp;nbsp; Health touches everyone -- and if the country spends $30+Billion dollars on 'investment' and the average citizen experience with the health delivery network is no different and costs continue to rise -- there is going to be accountability for where did the money go.&amp;nbsp;&amp;nbsp; She is rightly trying to focus the minds of the folks with the wagons -- better pick your spots wisely -- just getting a homestead isn't good enough -- it had better be productive land.&lt;/P&gt;
&lt;P&gt;My biggest ah-ha moment came a few hours after the event in a conversation with the CEO of a large player in the healthcare supply chain.&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; I realized that a significant portion of the Health Affairs event, the broader conversation about overall HIT investments and the specific conversation I was having ALL ACKNOWLEDGED and CONCLUDED that individuals (people, patients, consumers,&amp;nbsp; whatever label we want to use) have a right to a personally controlled health record and that the personally controlled health data management platform is a critical component (perhaps a necessary component) of ANY FUTURE HEALTH IT SYSTEM and probably any payment reform system.&lt;/P&gt;

&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;Here are a few of my observations from Asia Pacific—&lt;/SPAN&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;with more specifics this week to provide context:&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;Monday morning started with a visit to a large public hospital in Bangkok.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;They are at the same time very sophisticated from a diagnostic/therapeutic perspective (research lab, surgery center) and very antiquated from a work process perspective.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;They see a huge number of patients—both with appointments and not. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Their “system” is primarily paper-based—the amount of forms and paper they have is overwhelming. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Their whole system is based on queues—throughout the hospital, there are people waiting and standing in line. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;To get a pharmacy or lab item, a patient might go through three separate queues or tellers with an indeterminate wait time at each—identify/submit order, make payment, and draw or receive lab or Rx. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;I was struck by the incredible opportunity for the use of technology to improve the entire system—in terms of throughput and satisfaction, and ultimately patient care. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Hospital management also sees the need and the opportunity— to invest in the information infrastructure as they have invested in the medical equipment infrastructure—but the implementation task seems daunting to all.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;I spent the afternoon at &lt;A href="http://www.bumrungrad.com/" mce_href="http://www.bumrungrad.com/"&gt;Bumrungrad&lt;/A&gt; Hospital in Thailand where I have a development organization and key strategic partner. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Bumrungrad is an amazing example of excellence—using technology to support clear outcomes—quality, efficiency, and customer satisfaction.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Each time I’m there, they show me advances in infrastructure, workflow processes and facilities. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;It is great to have strategic partners who push the agenda for our products forward. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Now they’re moving on to Bumrungrad Version 3.0—a whole new level of service and are again looking to IT to help.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;While they serve the local population, they serve an even greater number of foreigners—thousands and thousands of “&lt;A href="http://en.wikipedia.org/wiki/Medical_tourism" mce_href="http://en.wikipedia.org/wiki/Medical_tourism"&gt;medical tourists&lt;/A&gt;” from the US, Middle East, Europe, and Asia.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Interestingly one of the key topics was how to leverage HealthVault in supporting the medical tourist and expatriate part of their business…something we have talked about internally but now have prioritized.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;In Kuala Lumpur, I visited two private hospital groups, each of which is part of a larger conglomerate.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;They view healthcare as a growth portion of their portfolio and they have money. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;I enjoyed learning from them about their goals and approach, and explaining our strategy to them.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;The perceptions of what Microsoft is as a brand are so prevalent and ingrained—consequently, I have learned my most important objective is to communicate with folks that HSG is a serious and thoughtful player in health information technology.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;These private hospitals want technology partners to help them do a lot; —streamline their clinical and business processes, use clinical workflow tools to put guardrails on clinical processes, provide sophisticated BI tools and to stay on the leading edge.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;While they are focused on their enterprise needs first—our Health connected strategy—of connecting consumers to physicians to facilities—really resonated with them for their future business needs.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;In Sydney, I met a very diverse group of folks—shadow ministers, MDs, CEOs of hospitals, members of the Clinical Excellence Commission, and members of the NSW health service.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Australia has been pursuing leveraging HIT for several years.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Some states are pursuing standardized Cerner implementations for hospitals and there is an institution named &lt;A href="http://www.nehta.gov.au/" mce_href="http://www.nehta.gov.au/"&gt;NEHTA&lt;/A&gt; that has been exploring the policy issues/standards around community or personal ehealth records.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;As I’ve noted before, the challenges in Australia are similar to the U.S. and Europe—leveraging information across the continuum of care, getting more out of their existing departmental systems, improving performance at the enterprise and system level and engaging consumers.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;The health of the healthcare system is a hot media and political topic—everyone is talking about the Garling report and what it means, physician concerns about the Cerner implementation in emergency departments and other specific bad ED incidents the media have publicized. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;While I get the scale benefits of standard software and standard rollouts by centralized and skilled service organizations—they frequently run into challenges in the healthcare domain.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;
&lt;P class=MsoNormal style="MARGIN: 0in 0in 10pt"&gt;&lt;SPAN style="FONT-SIZE: 10pt; LINE-HEIGHT: 115%; FONT-FAMILY: 'Arial','sans-serif'; mso-bidi-font-size: 11.0pt"&gt;I concluded my trip in Scottsdale, AZ, where I had the opportunity to address a small group of CEOs at the &lt;A href="http://www.hmacademy.com/" mce_href="http://www.hmacademy.com/"&gt;Health Management Academy&lt;/A&gt;.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;It was a great interaction about how technology has changed many industries. I had the opportunity to use my personal history to tell the story of how connecting vision, technology folks and domain experts has powered positive and sustainable change—which I have had the good fortune of doing at Microsoft, at MSNBC and at drugstore.com.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;It was fitting to conclude the trip back in the U.S with leaders dealing with the challenges in their organizations and the economic climate.&lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp; &lt;/SPAN&gt;Increasingly I see the local market separating into two segments—while all are focused on cost containment—some are primarily hunkering down and others are seeing the current climate as an opportunity to invest in transformation. &lt;SPAN style="mso-spacerun: yes"&gt;&amp;nbsp;&lt;/SPAN&gt;Obviously I believe the right technology is critical to both groups.&lt;o:p&gt;&lt;/o:p&gt;&lt;/SPAN&gt;&lt;/P&gt;

&lt;P&gt;Here are some observations worth sharing:&lt;/P&gt;
&lt;UL type=disc&gt;
&lt;LI&gt;Everywhere there is a deep interest in personally controlled health records - more than I anticipated, and despite the fact that some early projects in the category have performed poorly and have low adoption.&amp;nbsp; Stakeholders recognize that citizens/consumers need to be actively engaged in their health in order to improve the results and economics of the health system.&amp;nbsp; They view personally controlled health records as an important tool to motivate and engage them.&amp;nbsp; They don't see other options that can accomplish this goal. &lt;/LI&gt;
&lt;LI&gt;Privacy is a major issue surrounding the sharing of health data -- either in the professional arena (insurance, provider to provider, provider to pharma) or in the case of professional to consumer.&amp;nbsp; Many have concluded (as we have in the design of HealthVault) that personally controlled health records are the best solution (but not the only one). &lt;/LI&gt;
&lt;LI&gt;Health systems and their funders are looking for ways to connect the health delivery system across institutional boundaries -- from GP systems to hospitals and specialists.&amp;nbsp; No one is doing it well yet.&amp;nbsp; They see it as critical to delivering better outcomes and containing costs, and they acknowledge that seamless data sharing is critical to enable new work flows.&amp;nbsp; Unfortunately there remain many barriers to this seamless data sharing -- some technical, but mostly economic fear of existing stakeholders around loss of revenue or the implications of transparency on business practice/quality of delivery.&lt;/LI&gt;
&lt;LI&gt;The payment systems inhibit innovation in many cases or drive it in others.&amp;nbsp; In some markets, governments are pinning their hopes on private insurance as a vehicle for innovation and focus on wellness/prevention while in other markets the government is directly trying to tinker with the payment schemes to encourage new behaviors by the provider organizations.&amp;nbsp; From an economics perspective -- what strikes me as the real problem and challenge with these approaches is that the feedback loops are long and indirect, and consequently the cycle time of improvement will be incredibly slow.&amp;nbsp; This is why price mechanisms in the capitalist system are so important - real time feedback mechanisms with the ability to self adjust. &lt;/LI&gt;
&lt;LI&gt;The people with whom I met had an informed interest in what was going on in the U.S., particularly around the stimulus bill for health IT and about the prospects for health reform.&amp;nbsp;&amp;nbsp; They are watching to see the impact.&amp;nbsp; What surprised me was the low esteem in which most regarded the U.S. health system...based on the statistics around health spend as a % of total GDP spend and reported health outcomes.&amp;nbsp; I acknowledge that the U.S. system has many flaws, is broken in many ways, and has lots of waste that should be eliminated.&amp;nbsp; But I have always been a bit of a skeptic that the statistics on outcomes really tell the full and complete story around quality of care delivered across different societies.&amp;nbsp; I am not sure that the perception of others really matters in this case in terms of the U.S. needing to fix the problems regardless -- but it bothered me enough to flag it and put in my "think about it later" list. &lt;/LI&gt;&lt;/UL&gt;
&lt;P&gt;Some I visited are surprised to learn that Microsoft is investing in health specific software and services solutions.&amp;nbsp; Others are challenging us to do even more to bring ease of use, effective application integration and user interface innovation to the health worker desktop today!&amp;nbsp; Most are hopeful that the next round of technology and software investments really help improve the working environment on the front lines of health delivery and health outcomes.&lt;/P&gt;
&lt;P&gt;Next stop -- Asia Pacific.&amp;nbsp; Given our ambitious goals, I often tell my team that time is our enemy when it comes to success.&amp;nbsp; Ironically, I have turned the motivational saying into a concrete reality as my itinerary has three of the next six nights aboard planes.&amp;nbsp; whoops&lt;/P&gt;

&lt;P&gt;My main message to the Senate was:&amp;nbsp; We should really focus on the health outcomes we want to achieve, not just on the technology itself.&amp;nbsp; What the health system needs is to adopt technology in ways to deliver better outcomes, better chronic care management, better hospital effectiveness.&amp;nbsp;&amp;nbsp; We really want to make sure that we have the leadership focused on encouraging the usage of technology to achieve certain goals, like better chronic care management.&amp;nbsp;&amp;nbsp; &lt;/P&gt;
&lt;P&gt;We were the only technology company testifying, and I think people were surprised to hear us saying that technology isn’t the silver bullet.&amp;nbsp;&amp;nbsp; It's not that Microsoft doesn't want to see spending on health care technology. Far from it.&amp;nbsp; We just want to see smart spending, on technology that will really have impact.&amp;nbsp; When we decided to go to the moon, we didn't say let's build a great rocket...we said let's go to the moon...I feel a little bit of the conversation has been about let’s build a great rocket and hope we get to the moon.&lt;/P&gt;
&lt;P&gt;The other witnesses included Health Leadership Council President &lt;A href="http://www.fuqua.duke.edu/programs/duke_mba/health_sector_management/hsac/members/grealy/" mce_href="http://www.fuqua.duke.edu/programs/duke_mba/health_sector_management/hsac/members/grealy/"&gt;Mary Grealy&lt;/A&gt;, National Quality Forum President &lt;A href="http://www.qualityforum.org/about/leadership/" mce_href="http://www.qualityforum.org/about/leadership/"&gt;Janet Corrigan&lt;/A&gt;, Permanente Federation Executive Director &lt;A href="http://xnet.kp.org/kpinternational/faculty/cochran.html" mce_href="http://xnet.kp.org/kpinternational/faculty/cochran.html"&gt;Jack Cochran&lt;/A&gt; and Valerie Melvin, Director of information technology for the Government Accounting Office.&amp;nbsp; Their comments were unexpectedly aligned with mine -- namely take a holistic view; incenting the adoption of technology is not a silver bullet; and, we must have reform of the payment system too.&amp;nbsp;&amp;nbsp; So at a high level -- folks understand the challenges of creating the right kind of change in the complex health ecosystem.&amp;nbsp;&amp;nbsp; Where the differences lie -- is how to get started.&amp;nbsp;&amp;nbsp; Unfortunately as is often the case -- the stimulus bill (big incremental spend investing in health IT) is separate from the activities around health reform.&amp;nbsp;&amp;nbsp;&amp;nbsp; Getting the spend without the right payment system reform -- could lead to the unintended consequences the panel was cautioning against.&lt;/P&gt;
&lt;P&gt;The legislation is being drafted now -- that is why the hearing was held on a day when there was a lot of other activity going on in the Senate.&amp;nbsp;&amp;nbsp; There is a clear sense that something big will pass in 2-3 weeks - which is like moving at the speed of light.&amp;nbsp;&amp;nbsp; I am sympathetic to the challenges of the staffers trying to find the right language -- it is not easy to figure out how to guide the spend of $20billion over two years!&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; The scale of spend is still hard to put into perspective for me -- in my last post I tried a per physician number, so this time let's try per day -- it is $27M/day!&amp;nbsp;&amp;nbsp;&amp;nbsp; I am very confident there is lots of low hanging fruit to generate a return on investment in the health system (waste, overuse, misuse, error reduction) that technology can enable -- but still $27M/day!&amp;nbsp;&amp;nbsp; &lt;/P&gt;
&lt;P&gt;Since I spend the bulk of my time building software -- decisions about features, hiring great people, how and where to sell -- the hardest question from the staffers to answer is - how do you recommend we spend the money?&lt;/P&gt;
&lt;P&gt;My answer is in my closing remarks with the five key things that I think the government needs to focus on:&lt;/P&gt;
&lt;P&gt;1. Encourage innovation in health IT by setting out objective goals and criteria, not by mandating specific technologies or development models.&amp;nbsp; &lt;/P&gt;
&lt;P&gt;2. Reward innovative doctors who make the Internet the foundation of the patient-physician connection.&amp;nbsp; &lt;/P&gt;
&lt;P&gt;3. Provide incentives for sharing data.&amp;nbsp; &lt;/P&gt;
&lt;P&gt;4. Focus on making data interoperable today, not waiting for standards tomorrow, and insist that vendors separate data from applications.&amp;nbsp; &lt;/P&gt;
&lt;P&gt;5. Enable the private sector to develop an information infrastructure that connects data, systems, and people. &lt;/P&gt;
&lt;P&gt;These are really critical, so we don’t end up in the wrong place.&amp;nbsp; They’re based on our learnings as we’ve delved into this complex world of health.&amp;nbsp; &lt;/P&gt;
&lt;P&gt;One final thought -- surely it is an exciting time to be in the health information technology business!&amp;nbsp;&amp;nbsp; I personally got passionate about trying to really contribute and make a difference in HIT from my time on the President's Information Technology Advisory Council (PITAC) in 2003-2005.&amp;nbsp;&amp;nbsp; As co-chair of the Health Subcommittee -- we published a report titled &lt;A href="http://www.nitrd.gov/Pitac/reports/20040721_hit_report.pdf" mce_href="http://www.nitrd.gov/Pitac/reports/20040721_hit_report.pdf"&gt;Revolutionizing Health Care Through Information Technology&lt;/A&gt; with key recommendations for the government to consider.&amp;nbsp;&amp;nbsp; While my depth of understanding has gone up dramatically, what I find is both fascinating and perhaps cautionary -- is the discussion is largely on the same key issues/recommendations.&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; This time there is a lot of money at stake -- but it goes to show that change is hard and probably slow.&amp;nbsp;&amp;nbsp;&amp;nbsp; &lt;/P&gt;

&lt;P&gt;Yesterday, Microsoft hosted the &lt;A href="http://www.healthtransformation.net/" mce_href="http://www.healthtransformation.net/"&gt;&lt;STRONG&gt;Center for Health Transformation&lt;/STRONG&gt;&lt;/A&gt;&lt;STRONG&gt; (CHT) meeting&lt;/STRONG&gt; on our campus.&amp;nbsp;&amp;nbsp; CHT is led by Newt Gingrich -- he is a tireless champion for change and for raising our expectations for results from the health delivery system.&amp;nbsp;&amp;nbsp;&amp;nbsp; CHT does good work stimulating ideas, motivating policy makers to learn and be thoughtful when approaching health and creating connections between their members who are doing good work.&amp;nbsp;&amp;nbsp;&amp;nbsp; Newt was a judge on the HealthVault Be Well Fund panel.&amp;nbsp; Many U.S. ecosystem players participated and exchanged ideas -- sharing ways that IT plus leadership can improve overall results in the health delivery system -- like Gary Kaplan CEO at Virginia Mason, an integrated delivery system in Seattle that has utilized the "Toyota Lean" for system improvement.&amp;nbsp; Given the fragmented nature of the health ecosystem -- these stories of best practices and improved results -- need to be told and retold and retold -- to motivate additional champions how to change and give practical examples of how to make progress.&lt;/P&gt;
&lt;P&gt;The last two days I have been participating in the 4th annual &lt;STRONG&gt;&lt;A href="http://www.pacifichealthsummit.org/" mce_href="http://www.pacifichealthsummit.org/"&gt;Pacific Health Summit&lt;/A&gt;&lt;/STRONG&gt;.&amp;nbsp;&amp;nbsp; The mission of the PHS is to bring leading scientists together with policy makers and industry to tackle hard problems in Global Health.&amp;nbsp;&amp;nbsp; It is a fascinating conference because of the mix of people (scientists, ministers, NGOs, industrialists) -- all of whom bring a real passion to helping "improve" Global Health -- but start from very different perspectives and world views.&amp;nbsp;&amp;nbsp; This years theme (selected a year ago) was around nutrition and the twin challenges of under nutrition (hunger, nutrient deficiencies and their impact on health status) and over nutrition (obesity, diabetes and the impact on health status and costs).&amp;nbsp;&amp;nbsp;&amp;nbsp; The timeliness of the topic -- given current events -- only raised the level of urgency and passion behind the conversation.&amp;nbsp;&amp;nbsp;&amp;nbsp;&amp;nbsp; I won't even attempt to do justice to the theme in a brief blog -- given the complexity and interdependencies of the key threads -- I'm sure a final report will be published.&amp;nbsp;&amp;nbsp;&amp;nbsp; I can report a few surprising data points for the benefit of others:&lt;/P&gt;

&lt;LI&gt;despite the lowest incidence of obesity -- Japan implemented a national policy in April to focus on prevention because of current costs and the need to change trend lines. This means that all people in Japan ages 40-74 are required to get an annual checkup.&amp;nbsp;&amp;nbsp;&amp;nbsp; One panelist -- Takemi-san a former vice-minister of health -- described how he had made a public promise to lose 10 kilos - -and this became a big and frequent media event (waist measurement and weigh in).&amp;nbsp;&amp;nbsp; But he achieved the goal!&lt;/LI&gt;
&lt;LI&gt;China too has started a new program to prevent obesity -- as part of an overall approach to public health.&amp;nbsp;&amp;nbsp; The program's essence is its simplicity -- healthy weight and healthy blood pressure.&amp;nbsp;&amp;nbsp;&amp;nbsp; They determined that if they could motivate everyone to eat 40kcals less/day -- they could dramatically change future trend lines.&amp;nbsp; So they ask people to eat 2 less bites off of their dinner plate.&amp;nbsp;&amp;nbsp; &lt;/LI&gt;
&lt;LI&gt;India -- while being a food exporting nation -- has roughly 40% of children under 5 being under-weight.&amp;nbsp;&amp;nbsp; This data point was used to demonstrate that "point solutions" or "simple programs" don't work -- that behavior change at either the national or individual level require comprehensive approaches (education, training, incentives, etc) that also take into account culture and traditions.&lt;/LI&gt;&lt;/UL&gt;
&lt;P&gt;The role of nutrition in Global Health is clearly important -- and impacts costs and outcomes throughout the system -- from childhood development through to the management or prevention of chronic conditions.&amp;nbsp;&amp;nbsp;&amp;nbsp; One key takeaway for me is that it will require leadership (at multiple levels) and new forms of collaboration between the private sector and the public sector to sustainability address this issue.&lt;/P&gt;&lt;img src="http://blogs.technet.com/aggbug.aspx?PostID=3074051" width="1" height="1"&gt;</description><category domain="http://blogs.technet.com/neupertonhealth/archive/tags/HealthVault/default.aspx">HealthVault</category><category domain="http://blogs.technet.com/neupertonhealth/archive/tags/global+health/default.aspx">global health</category><category domain="http://blogs.technet.com/neupertonhealth/archive/tags/obesity/default.aspx">obesity</category></item><item><title>Reflecting on Progress</title><link>http://blogs.technet.com/neupertonhealth/archive/2008/06/08/reflecting-on-progress.aspx</link><pubDate>Mon, 09 Jun 2008 00:26:52 GMT</pubDate><guid isPermaLink="false">d5e57398-b9ef-4490-9955-07cbb4e4a80d:3067803</guid><dc:creator>pnblog</dc:creator><slash:comments>0</slash:comments><comments>http://blogs.technet.com/neupertonhealth/comments/3067803.aspx</comments><wfw:commentRss>http://blogs.technet.com/neupertonhealth/commentrss.aspx?PostID=3067803</wfw:commentRss><description>&lt;p&gt;We are hosting the second HealthVault Solutions conference June 9 &amp;amp;10 in Bellevue.&amp;#160;&amp;#160; The amount of effort and activity on our part -- and on the part of our partners who will showcase their web applications at the event is truly remarkable.&amp;#160;&amp;#160;&amp;#160; As a startup guy most of my career -- this infectious enthusiasm and passion by all the folks involved -- generates the endorphins that make startups and innovation so much fun.&lt;/p&gt;  &lt;p&gt;For those that know me -- my primary focus is always on the next problem, on identifying and working on the things we can and must do better and not looking back at the accomplishments or progress.&amp;#160;&amp;#160;&amp;#160; As I prepare for this event however -- what really strikes me is the progress we, along with our partners (and others) are making towards the vision of empowering consumers and physicians to use the Internet effectively as a tool to improve health.&amp;#160;&amp;#160;&amp;#160; &lt;/p&gt;  &lt;p&gt;A year ago -- we had working code and a beta environment for our very early partners.&amp;#160;&amp;#160;&amp;#160; Nothing available to the public.&amp;#160; Lots of talk and ideas here and in the industry at large - but not much tangible.&amp;#160;&amp;#160; Today we have over 30 web health applications that copy data to/from HealthVault and enable users to reuse their health information in new ways without the burden of re-entering it all the time.&amp;#160;&amp;#160;&amp;#160; We also have 50 personal health devices from weight scales to glucometers to peak flow meters and more that connect directly to HealthVault -- which makes it so much easier for people to use this information to understand how to stay well or to use it in communicating with their physician.&amp;#160;&amp;#160;&amp;#160; In addition to our efforts -- Google Health is also now available to consumers.&lt;/p&gt;  &lt;p&gt;Because of how HUGE the health eco-system is -- trillions of dollars, hundreds of millions of consumers/patients, hundreds of thousands of physicians, thousands of hospitals, hundreds of insurance plans and so on -- progress can be hard to notice in the early phases.&amp;#160;&amp;#160; First -- we can celebrate the many partners that saw the vision early and have already built connections with HealthVault and we love them.&amp;#160;&amp;#160;&amp;#160; But I can also see a big difference in the conversation we are having with stakeholders in the eco-system and it is evolving from the tire-kicking phase to the &amp;quot;how can I get involved to help my business or my customers&amp;quot; phase....which to me indicates momentum is building and building fast.&lt;/p&gt;  &lt;p&gt;Every once in a while it is nice to look back and recognize that the vision we have around HealthVault -- of building a consumer-centered health platform -- which lots of stakeholders will use to help consumers and themselves -- is really working!&amp;#160;&amp;#160; Now, back to working on the things we can do better. &lt;/p&gt;&lt;img src="http://blogs.technet.com/aggbug.aspx?PostID=3067803" width="1" height="1"&gt;</description><category domain="http://blogs.technet.com/neupertonhealth/archive/tags/HealthVault/default.aspx">HealthVault</category></item></channel></rss>
